INSULATION & AIR SEALING Mass Save Home Performance Contractors Information Form Home Performance Contractors (HPCs) have met training and certification requirements and provided proof of insurance in order to become a participating contractor of the Mass Save Home Energy Services Program and provide Home Energy Assessments and weatherization services. Pricing for air sealing and insulation improvements is set by the Mass Save Home Energy Services Program (“the Program”) and will remain the same, regardless of the contractor you use for qualified improvements. The HPC you selected may offer non-Program related recommendations for additional energy related equipment and/or services such as: • Heating Systems • Heat Pumps • Solar Systems • Roofing • Air Conditioning • Water Heating Systems • Siding • Windows, and more PLEASE NOTE, these services are NOT associated with the Program and may require additional payment of which pricing is NOT set by Mass Save. All pricing, contracts, or payment terms associated with non-Program work are between the customer and the HPC. You have the option of working with any licensed contractor of your choice, which does not impact the rebate eligibility for qualifying equipment. The Program does not endorse or warrant any specific contractor or any work conducted outside of the Program. It is suggested that you solicit multiple bids for any services beyond air sealing and insulation. By signing below, you acknowledge receiving this disclosure form. You confirm that the Home Performance Contractor conducting your Home Energy Assessment explained all the details on this form and that you have the described options, rights, and privileges within the Mass Save Home Energy Services Program.
